Geek Squad Service Request

  • Allows customer to replace a device under the Geek Squad Protection Plan
  • Supported devices associated with the service are iPads, iPods, and cell phones
  • Customers are required to provide the Geek Squad Protection number

The Geek Squad service request program allows customers to check on a current service request or create a new request.  To create a new request using the Geek Squad service request program the customers will have to provide their name, Geek Squad Protection (GSP) number or the e-mail address used at the time of purchasing the Geek Squad Protection plan.  Those who forgot the email address they used when making the purchase will have to go through a bothersome recovery process (it is best to simply use the GSP number if available).

Geek Squad Service Request Comments

Please NOTE that Geek Squad Protection Plan excludes damage resulting from abuse, cosmetic damage that does not affect functionality (customers are not allowed to obtain a replacement just because the device has a scratch on it), devices completely submerged in liquid (tossing a phone in a swim pool for example), and software issues.

My Exam One Results Register

  • Register to receive My Exam One results online
  • Registration with Exam One should take less than 5 minutes to complete and is free
  • Patients can expect to receive the laboratory results within 7 to 14 days subject to the patients insurance company’s participation

My Exam One results register requires a valid email address (please use one that the patient has access to as they will need to confirm it) and please note not all insurance companies will partipate in the My Exam One results service.  Those who do not participate in this program most likely already have another system in place to deliver exam results online.  My Exam One users who receive a prompt that lab results are not available should contact their provider or insurance agent to inquire about the availability of laboratory results.

Exam One has more than 700 locations across the United States and any questions about the service can be directed to a My Exam One customer service agent at 877-933-9261.  Most exams are part of required testing due to insurance reasons.  All patients will have to  read and accept the Terms of Use of the My Exam One service before results are provided online.
